How do install this form script? $NP reward

Hi guys,

I found this Ajax form script but i have no clue how to install this.
The zip file does not contain a read me or help file with installation instruction so i have no idea where to start or to set this up.

Umm, just upload the files to your website and it'll work. If you want to put your own page design around the forum, simply include all of the javascript files and stylesheet data into your template and include it around all the HTML stuff in index.php (all the main HTML after <body> and before </body>)

I don't want to sound ungratefull to your response but that really did not clear things up for me

I mean there are various files in the zip file...do i just plunge everything in the root directory? Where the index page resides so to speak.
I'm really not that experienced with coding or server administration so what may sound obvious to you is totally new to me.

These are the files in question:

A file named: img
A file named: scripts containing contact.php and xmlHttpRequest.php
A file named: js containing three javascript files named

contact
functionAddEvent
xmlHttp

A file named: index.php

I dont want the contact form on the index page since i have other content reserved for this.
So how would i go about this?

Upload the folders found in the zip file as they are. So they would be located off your root folder.

<root>/img
<root>/scripts

Then rename the index.php file in the zip folder to whatever you want, say contact.php

Upload contact.php to your root folder and it'll work.
