[HELP] Editing Header with .htaccess

Ok does anyone know how to do this? To like add a code to all pages? I think it is with the append feature, but could I get an example... I do not know how to insert it correctly..

but the member/user can't access the .htaccess once he/she has access to it, he/she just delete your ad, you need to modify Apache Module mod_ext_filter to insert forced ads (header or footer or both) or much better invest on xpanel which does this for you and whole lot more, http://www.xpanel.com/
