Header Wanted -- $35 {contest}

I am looking for a header image for a website.. (image format)

The header should include:
A search Bar.. (Find (cat) within (miles) of (zip)

Links: 4 black link areas

PSD file.

Dimensions: 778 x 100-200

I would like it to follow this color schene on the following logo.. I would also like the logo used if possible.

I dont know what you mean by a search area with that text and stuff.
