Feedback application

Can you recommend some simple application for collecting feedback from website visitors? I would like to put it in my website and get visitors opinions on how my website works. I can not find anything interesting yet .

You could probably write one yourself...you just need a basic submit form and optionally, a captcha to weed out spam bots.
