Custom Name Product Script needed

Hi there,
I am in need of a custom name product script needed. It basically needs to allow users to type their name in a text field, add some custom shapes, e.g hearts/skulls etc and have a dynamic preview. The name that the user enters will then be processed and passed to the shop owner via PayPal.

Budget for this is $50-$100.
