Best Books to learn PHP5/MySQL & HTML5/CSS3?

I wonder if anybody has any recommendations for books for learning PHP5/MySQL & HTML5/CSS3?

I would say I'm a basic to intermediate programmer for HTML4/CSS and have little to no real knowledge of PHP5/MySQL. I learnt my HTML from a Visual QuickStart Guide.
