I have attached a preview of a design I just did. Four psds included:

Main, Image upload, Movie Upload, and File Upload. Not coded, but could be easily integrated into any upload script.

full Resale and ownership rights included, will never be used again and will be deleted off of my HDD forever. All I keep is a jpg thumbnail for my records.
