How to post using "Get" method

Hi,
I want add post the following instructions from my form, using the "Get" Method

I need to post the following:
http://www.otherdomain.com/post.aspx?Email=&IPAddress=[IP]&DateTimeStamp=[DATE]

Can I add this on my existing form page? also how do put a check box so it only submits this if the box is ticked.

To send using get method, change the method to get:
example:

<form name="MrForm" method="GET" action="processpage.php">

</form>

in regards to the checkbox... I would just put in a checkbox and see if it was ticked in the next page. If it wasn't ticked send the user back to the form again and tell them to tick it.
