Forms stopped working...help!

Hi all,

The forms on my website have suddenly stopped working. An example of the form can be found by visiting:

Any advice on how I can get this working would be greatly appreciated.

its a 404 error

has the server config changed? looks like your using frontpage extensions and looks like they have been unisntalled for some reason. or disabled.
