Creating an interactive survey with PHP

I'm "designing" (yes, in fact I don't know that much of html, etc.) a website on 'skincancer'.
One part of the website is an interactive "test" where visitors can find out what skin-type they have.
Therefore they need to answer a couple of questions with 4-5 different answer-possibilities. Every answer has its own "weight" and based on the sum of those "weights" the skin-type is attributed.
I've seen this kind of tests on other websites and there they use PHP or Javascript.
I'd prefer PHP as Javascript isn't very 'stylish'.

Problem is: I know "what" PHP is and what it means, but I can't put together one line of PHP-code...

My question: anyone inhere who could code this test for me ?
If so, please PM me and let me know your asking price (if you don't mind, feel free to post your price in this thread).
Preference will be given to someone who has already coded a test like described above.

Anyone who knows of a program/script that can be used to do this job myself ? (I already found TONS of PHP-quiz-scripts, but that's not what I'm looking for)
