Because of the high commercial value for this combination and its previous use by an actual company, I'll give it low $$$$ on the high end, with mid $$$ far more likely. If the other extensions were taken, I'd be inclined to think it is worth even more.

I get your rationale, but I do not like this extension at all. And healthcare names are also harder to sell from my experience. So with that being said, expect to get low $$$ on each, and if you get lucky and outbound one to some rich doctor who doesn't know much about domains, maybe low $$$$.
