PayPal Form Script

Hey all,

I'm looking for a script that will allow me to set up a regular form; that will essentially have a 'donate' button as a submit button.

My goal is to collect information about a user, and collect an entry fee at the same time.

Does it allow for full customization and addition of form objects; that's the question

You could ask the creator if it's possible to do customization.
Btw After collecting the information and receiving the donation, where would it direct the user?
