Hi Every body!,

I have a problem that i really need all of your help!if any can help me or give me the layout please give.

As i was supposed to buil a small website to solve the task of A Request Form Procedure in my company I call "IT Mantenace Request Form Procedure"
Let me describe the IT Mantenance IT Request Form Procedure First.
1.Purpose of this IT Mantenace Request Form Procedure:
this is a task of an employee in the company,when she or he has the problem with the computer or printer or CD rom forexample She has to make an IT Mantenace IT Request Form, this is a paper, she or he has to fill in the request form his or her name,Extenision number,the place where the problem occurred
then this request goes to the his or her supervisor and wait to prove , the supervisor decides whether that request is accepted or not if accecpted this Request goes to IT TaskCenter if not this request goes back to the people who has made that request.
2.When the request is accepeted it goes to IT Taskcenter then the Admin of IT TaskCenter checks this request, and assinged His IT Task Center Users to go and fix that problem in the request,if the TaskCenter User can fixes the problem in the request he closes that request then the people who made that request has to prove done request on the IT mantanence Request Form then this request finishes, if the TaskCenter IT Users can't fix this problem on his shift he has to send this job to other shift then the people who on next shift has to continue to solve the problem on the request.

Please help me with this.
Note:
-have an IT Mantenance Request Form on hand by paper then i wil send this Request Form then it will be easily to see my problem.
-I also designed the Data Flow Diagram for this Request procedure.
Right now i don't know how to do next.

Please help me any word will be very appreciated!.

if you want the simplest, easiest cure.... just do a secure webpage (https://)
and use a mailto: form, its really simple. you can have it mail to the sup, then all he has to do is mail it to the IT manager who can then do what they do, however, the whole things seems pretty beurocratic to me, thats far more work than is needed.
