Need form data to save to MySQL

I am in need of a form system that will save its data to a mysql db, and be retrievable from there to display on another page...

I want it to be able to have the users type & select information on the form, submit it, and then other members click "View Others" and a list of names that have submitted information will be display'd... Clicking on some1's name will display the inputted data from the form.

I will go more into depth with you if some1 is intrested.

I am not too proud to sign a NDA for a $20 job. It takes like 2 minutes to sign and fax, woopity-do. $20 is $20 afterall.

Okay, scratch the NDA, and it is something simple, in thoery ... What I want the system to do is really complex and hopefully some1 is intrested. I am lookin to get some1 going on this project pretty quick. And them to be able to work with me over the course of a week or so to get the system going to my exact desire. I wanted an NDA because I wanted this system to be unique, but I guess (no offence intended) no1 wishes to fill that part of the request, and my despratness to get this project finished is imperative. I am way behind my personal deadline of the 31st of January considering it is today.

yes it doesnt take that long. And I have really thought bout this plan and I am pretty sure its gonna cost me more then 20$. Which is fine because I want it to have the features. And the oriinal intention of the NDA was to allow me to develop (with ur help) and ioncube it and resell it. Ad with the source code floating around...

Well, it would be our "loss", not "lose". Well you could have sent it to me, but offering $20 doesn't make me jump for joy. If you need a good developer, don't offer a price, ask for a quote.
