I need a script to use for my school yearbook. Every year we have an index for the graduating class in the back of our yearbook so people can keep in touch. I need a script that can collect the information for each student, generate a list with this info, and allow me to simply copy and paste the info into the yearbook editor.

I just want a nice simple interface with the information fields, and then a submit button. Then I'd like any info submitted there to show up on another page alphabetized by last name with the person's name bolded.

For instance

Last Name, First Name
Address
Email
In Ten Years I See Myself:
blah blah blah blah blah blah blah blah blah blah blah blah blah blah
Activities: (Limit to Four Entries)
-blah blah
-blah blah
-blah blah
-blah blah
Favorite Quote:
"blah blah blah blah blah blah blah blah blah blah blah blah blah blah"

So yea about how much would this cost? This quote will be passed on to the yearbook manager to be approved as a yearbook expense. Thanks
