Need a PHP form modified.


Need a form that was generated by SourceForge.net PHP form generator modified, or a new form to replace this one. The current form is at: http://www.mortgage-match.us/prequal/forms/form1.html. We need to add 4 or 5 new fields to this form, plus incorporate a feature that does a mathematical calculation. The calculation would be based on this premise...

as you can see this form pertains to mortgages... one of the existing fields is "property value". A new field added would be "Loan amount applying for". The loan amount number input into this field will calculate to a percentage, or what's called Loan-To-Value (LTV) ratio, of the number input into "property value". As an example, let's say the number input into "property value" is $200,000. And, the number input into "Loan amount applying for" is $100,000. In this example, the LTV would be 50%. Because, $100,000 is 50% of $200,000.

I know very little about PHP or any code that would solve this need. So, I'm not if what we're looking for is possible. If you can do it, please provide me with an estimate, or quote, and a timeframe for completion. Thanks! We'll compensate through PayPal.
