Very Basic Project $$

I need a very basic project done for me...

First, I need a simple web design with a header, body, and footer in an 800px wide area. I'd like it to be simple, but with a CSS-refined look to it. Feel free to use any legal sources you want for ideas, like OSWD.org or any free templates with no restrictions.

Secondly, I need a relatively simple database application (PHP/MYSQL). Basically, I need an input area which will have these fields: Symbol, Entry, Target, Stop. These will then be put into a database. I then need a script to draw the symbol with the most entries and output the symbol, average entry, average target, and average stop (which are all numerical).

So, if anyone is up to this, let me know your price and email address via a reply or by PM. I'd like to get this done tonight if possible. Payment via PayPal. Also, post or PM me any questions you may have...

Well, the developer I usually use said it would take him about an hour to do (so I assumed it was pretty basic), but he doesn't get in until Monday... I was hoping to get this done tonight or Sunday.
