You write "Secure Login".
It is not secure at all, because you don't use https.
Better telling "Login" instead of "Secure Login".

And its validated!!! You know your stuff, Nice job, looks sleek and clean. Seems a little empty for some reason. Although I don't know what you could add. Maybe make the table a tad bit bigger, and lighten up the background a tad bit. Thats my opinion, but if you leave it the way it is, It still looks great.

the email form is just the php mail() function with the x-header "FROM: $email". The php would look like this:
