PHP - Need Script - $$$

Hello.

I am looking for a programmer to code me a *custom* PHP form script. I am hoping that this will be an easy and cheap job.

I want the PHP script to be very basic, however, easy to edit. I need a form so that people can submit their site links for affiliation with one of my sites. Of course, I want more than one text field.

I would like the results of each form submission to be stored online. I would like to view the results online using an administration panel. From the administration panel, I would like to be able to set if I would like the results to be emailed to an email address, or stored online...or of course, both.

If anyone is up for the task, please reply back here. I would like this done fast, too. I want to be able to use this script on different websites, so please be sure that forms are either easy to edit the form fields with basic coding, or editable in the administration panel.

Thank you. If you're looking for a rough-sketch of my budget, it is $20. I am not hoping that this script would be that much. I think it is a basic script...

I can do this for you it is a simple task. 20$ would be fine.

I do have AIM DubTheWorld.

I can do it. By simple task I mean it won't take that long to do.
