creating an interactive survey with PHP

I'm "designing" (yes, in fact I don't know that much of html, etc.) a website on 'skincancer'.
One part of the website is an interactive "test" where visitors can find out what skin-type they have.
Therefore they need to answer a couple of questions with 4-5 different answer-possibilities. Every answer has its own "weight" and based on the sum of those "weights" the skin-type is attributed.
I've seen this kind of tests on other websites and there they use PHP or Javascript.
I'd prefer PHP as Javascript isn't very 'stylish'.

Problem is: I know "what" PHP is and what it means, but I can't put together one line of PHP-code...

My question: anyone inhere who could code this test for me ?

I would love to create your survey and I have lots of knownleadge in both HTML, and PHP if you wish to know mroe about me please ask PremiumHostedNET or CoolDude as they will tell you I am the man for the job.

My invoice would be the following.

2 Days (Roughly) PHP Coding => 6.00$ (US) or 90NP$ Per Day => 2 * 6.00$ or 90NP$ => 12.00$ (US) or 180NP$
