Also you don't want a high CTR it will throw up red flags from google... keep the traffic high and the ctr down and you should be good to go

the skyscraper ads work best for me, i think they're more effective than they box ads.

Ad Format depends on your site layout. I don't like put those big box ads on top because it gives your site a look of MFA site.
