VERY simple PHP mail Form needed

Basically, I'm real busy and not that good at PHP.

I need a page converted from HTML into a PHP mail for sender.

I know it's easy, and will only take someone 10 mins to do it.

Bascially, this is the page : http://www.stuffandthings.net/qotm.html

It's for my step son in the US. If someone could do it within the hour, i would be MOST gratefull.

I'll give whoever can convert it $NP 300 immediately.

Perhaps you could modify or use this form, someone offered them for free and I downloaded them, www.Freebie.ws/forms.zip they are more geared towards a Hosting order form but I think you can get it to work for your project.
