Pretty good list. Obviously could jockey them around a bit depending on personal preferences.
I'd say I'd expand on the advertisements. It's not fully a popup, but the interactive ads that cover half the page that you cannot always close get really annoying. Another problem I notice is websites with very poor navigation.

Heres one that should be added to the list, those ads that pop up in the middle of the page, with the x on them to close it.
Do you press it or not press it. Some cooperate and seem to go away, others just pop up a bazillion other ads. THEN doesnt clicking on the ad in anyway give it the ability to install or do whatever other malicious things it wants?
I'm guilty of the advertisement overload with regfee.info, but just because I've been too lazy to remove all of the premiumdrops ads (heck, I'm too lazy to even post there that often).
When I first got into web design, I remember making this really cool javascript powered midi jukebox for my banner design business' website. I was the coolest 14 year old on the block.
Those new pop overs are very annoying. Especially when they play a video or something. And they follow you as you scroll. Finding the X to close them isn't always easy and they don't always shut down when you do click.
I can't deal with fluorescent yellows on pages (but I like bright lime green for some reason).
I also can't stand sites that tell you what your IP is (esp. if it's not an IP-checker site), all of a sudden it's like there are 10 spy-cameras around me or something...
